If the patient gives consent to move forward with the application process you will need to have them sign a Payment Plan Application Questions form (located in the middle cabinet, on the bottom shelf on the right hand side of the cabinet.). On the form are questions we will need answered, to fill out the application for the patient. You will need to scan a copy of the signed filled out form into the patients images in open dental under the financial tab. Once it is saved you can shred the card.
Once this is filled out, let the patient know you will go begin the application and will return once you are done. You will take the card and go to a computer and pull up the care credit website. Once logged into the providers portal click “New Application”.
